OCIA Registration

Welcome, and thank you for your interest in learning more about the Roman Catholic Church. We are honored to accompany you as you begin this journey of faith and will do everything we can to support and encourage you along the way.

As a first step, we ask that you complete this application questionnaire. The information you provide will help us get to know you better and understand your individual circumstances so that we may serve you more effectively throughout the OCIA process.

While this questionnaire may seem lengthy, each question has been included for a specific purpose. Some questions may not apply to your particular situation; if so, please leave those sections blank. For questions requiring a "Yes" or "No" response, please check the appropriate box.

If you encounter any questions that you do not understand or do not feel comfortable answering, you may leave them blank. These matters can be discussed later with the pastor or the OCIA Coordinator.

After your completed application has been received, an appointment will be scheduled with the OCIA Coordinator to discuss the next steps in your faith journey.
